What Is A Family Health Team?

A Family Health Team (FHT) is an approach to primary health care that brings together different health care providers to co-ordinate the highest possible quality of care for you – the patient.

Healthcare teams comprise professionals from various disciplines who enter a collaborative relationship with the patient to deliver coordinated, high-value, and patient-centered healthcare.

The Superior Family Health Team is a part of the Sault Family Health Organization.

Who Is A Part Of The Family Health Team?

Clinical care teams usually include groups of physicians, nurses, physician assistants, clinical pharmacists, social workers, and other health professionals that help to establish new lines of collaboration, communication, and cooperation to better serve their patient’s needs.

Established & Serving Our Community Since 2010

The Superior Family Health Team recognizes the importance of partnering with community members and partner organizations to ensure the success and sustainability of the proposed program.

The SFHT provides team-based support with supplies, staff, decision making and financial support with mental health allocation for community events and initiatives.

As such, we have an ongoing partnership with the Algoma Ontario Health Team (AOHT) with the goal of involving patients, their families and caregivers in the design and development of programs and services that align to the local priorities of an integrated health system. We also collaborate with Harvest Algoma, to address food insecurity issues affecting our patient population.

Professional To Professional Support

Designed to give doctors support from other complementary professionals, most Family Health Teams will consist of doctors, nurses, nurse practitioners and other health care professionals who work collaboratively, each utilizing their experience and skills so that you receive the very best care, when you need it, as close to home as possible.

Wheelchair Accessible

Our office is wheelchair accessible and meets AODA standards. If you need additional support, please call the clinic before arriving.

We Do Not Discriminate

The SFHT provides a safe accepting space for our 2SLGBTQ+ community with signs and posters for community events and inclusion.

The SFHT also provides 1:1 support social work for First Nations Culture-Based with Stephanie Stephens, MSW, RSW Anishinaabe, Mukwa Dodem. There is a poster in patient wait areas promoting the opportunity to work with a culturally based social worker.

We are also proud to host a Two Row Wampum belt in our front lobby to assist in the healing relationships with our First Nations Community. There are 11 First Nation Communities within Algoma. This is brought to events, discussed at reception and even borrowed by other organizations for teaching purposes.
