Memory Clinic

The SFHT provides a team-based Memory Clinic for community as well as our rostered patients.

Our Memory Clinic provides comprehensive assessments for patients with memory issues, including:

  • A review of the patient’s medical history, medications and test results
  • Assessments for memory, cognition and depression
  • Driver assessments
  • Caregiver stress
  • Review of current community supports and identify any gaps
  • We diagnose, recommend treatment, provide referrals for community supports and provide follow-up as appropriate.

This is a collaboration with SFHT clinical staff and the Alzheimer Society.

The Breton House

Also called The Algoma Substance Abuse Rehabilitation Centre.

Women enrolled in this program are from outside of Sault Ste. Marie, and do not have access to a primary care provider while living at Breton House. The SFHT provides care for the course of treatment (usually 1 year) decreasing SAH ED visits for needs such as preventative screening and prescriptions.

The Community Wellness Bus Program

The SFHT is a founding partner with Canadian Mental Health, Ontario Health Team, Sault Area Hospital and Algoma Social Services to provide primary care services to community member living with mental health and addictions and unhoused population.
