A Registered Social Worker who is specialist in mental health that provides patient-centered assessment and treatment services to patients enrolled with a physician at SFHT. They work independently and as a member of an interdisciplinary team and takes a lead role in providing clinical assessment, treatment and educational services to the patients of Superior FHT, consistent with the organization’s overall goals and objectives. They contribute to the ongoing development, implementation and evaluation of structure, policy and capacity within Superior FHT to meet the goals of our patients. They possess excellent organizational and communication skills.
The Social Worker will share their expertise and knowledge of health promotion, disease prevention, and chronic disease management across the lifespan of Superior Family Health Team clients. They will determine the impact of the patient’s mental health status on the risk for chronic disease or management of chronic diseases.
Treatment/Management/Planning
The Social Worker will develop individualized, patient-centered care plans based on comprehensive assessments incorporating related social determinants of health and best practices. They work collaboratively with the interdisciplinary team, family members and community agencies where appropriate, for the purpose of consolidating and coordinating patient treatment.
They evaluate the achievement of client goals through follow-up visits.
They also use information obtained from screening SFHT clients to assist with program planning to best meet the needs of the client.
The Social Worker is to practice within the scope and according to the standards of practice as outlined by the Ontario College of Social Worker.
