As a dietitian working in a FHT, we see patients with multiple conditions and comorbidities. The beauty of the FHT is that we can give truly individualized care that encompasses medical issues, socioeconomic drivers of health, patient values and preferences. Our Dietician is a certified diabetes educator with 17 years of experience and is part of the diabetes program that, together with the pharmacist and NP, works using medical directives to provide the best integrated care.
A typical day for our dietician consists of varied physician-generated and self- referrals. Common conditions seen include gastrointestinal issues, food allergies, diabetes prevention, hypertension, dyslipidemia, post-bariatric surgery care, malnutrition and frailty, wounds, sports nutrition and many, many more.
Our community exists in a dearth of both primary care and outpatient nutrition services. Our Dietician is one of only two outpatient RDs that service general (not disease focused) nutrition care in the whole of our city of 75,000 people.
