The pharmacist works collaboratively with other members of the multidisciplinary team to provide patient centered care and optimize drug therapy outcomes. The clinical pharmacist is responsible for leading the smoking cessation and the anticoagulation programs. They provide medication reconciliation services after hospital discharge to all patients of Superior Family Health Team. The pharmacist is also involved in monthly MINT Memory Clinics and the Diabetes Program. The pharmacist conducts medications assessments on a consult basis to address polypharmacy and to review complex drug regimens with primary care providers. They research and answer drug information question from other health care providers and patients. The pharmacist participates in patient education of chronic disease and medication counselling. They liaise with community pharmacists regarding patient’s drug regimens. The pharmacist supports and assists with drug coverage applications to provincial and private drug plans. The pharmacist is involved with teaching pharmacy and medical students that are working at the clinic.
