The Superior Family Health Team’s Strategic Plan reflects a strong commitment to expanding equitable, community-rooted primary care across Northern Ontario. Building on over a decade of growth since its establishment in 2010, the plan leverages recent Interprofessional Primary Care Team Expansion Funding to strengthen patient access, expand services aligned with community needs, and invest in workforce sustainability, technology, and communication. Developed through a thoughtful and collaborative planning process with staff, leadership, partners, and community stakeholders, the strategy prioritizes enhanced staffing and infrastructure, improved care coordination, and internal capacity building. Guided by compassion, innovation, and equity, the Superior Family Health Team is positioned to continue delivering high-quality, team-based care while responding to the evolving health needs of the Sault Ste. Marie and Algoma communities.
